What we collect and what for
Through the form on this site we collect only:

We do not ask for any information about your health or your hearing. How you hear is something we talk about at the centre, in person. If you voluntarily write any detail about your health in the comments field, we will treat it with the same confidentiality, but we ask you not to: it is not needed to book an appointment.
There is only one purpose: to get in touch with you to arrange an appointment. We do not use your details to send you advertising and we do not pass them on to anyone else.
Why we are allowed to use them
The legal basis is your consent (art. 6.1.a RGPD, the Spanish name for the GDPR), which you give us by ticking the box on the form. You can withdraw it whenever you like, and withdrawing it does not affect what was done before.
How long we keep them
We keep the details from the form for as long as we need them to deal with your request. If you do not go on to become a patient, we delete them within a maximum of one year. If you do become a patient, your records are kept in line with the applicable healthcare legislation (Ley 41/2002), for a minimum of five years from the end of each episode of care.
Who else can see them
- The provider of the form service, with servers in the European Union and a signed data processing agreement: Brevo (Sendinblue SAS)
- The website hosting provider: Vercel Inc.
- The centre you choose, so that it can look after you
No international transfers of data outside the European Economic Area take place.
Your rights
You may exercise your rights of access, rectification, erasure, objection, restriction of processing and portability at any time, by writing to akustikjavea@gmail.com or by coming to any of our centres with a document proving your identity.
If you believe we have not dealt with your request properly, you can complain to the Agencia Española de Protección de Datos (the Spanish data protection authority) (www.aepd.es).
Minors
This site is not aimed at children under 14. If you are under that age, ask an adult to fill in the form for you or call the centre directly.
Security
We apply reasonable technical and organisational measures to protect your data. The connection to this site is encrypted (HTTPS).
